The San Antonio Fire and Police Pensioners Association is proud to offer Dental and Vision Insurance to ALL members of the San Antonio Fire and Police Pension Fund.  
This page contains links to the forms needed to enroll with QCD of America, Inc. You can also obtain the QCD of America Inc. enrollment form along with details of the offered plans at http://safppa.myqcd.com. This site does not contain the payroll deduction form needed. The payroll deduction form is at the bottom of this page or can be obtained at the Fire and Police Pension Fund office at 311 Roosevelt Ave.

ENROLLMENT PROCEDURE

Below are the links to the details and information for all the available plans. Also the link to the QCD of America enrollment form and the Fire and Police payroll deduction form. Select the plan that fits your needs. Print and complete the QCD enrollment form. Then print and complete the Fire and Police Pension Fund payroll deduction form. Double check your forms for errors. Then mail ALL forms to QCD of America, Inc. at the address shown on the enrollment form. QCD will enroll you in the plan you selected then mail the payroll deduction form to the Pension Fund Office. There will be a 30 to 60 day wait for the plan to go into effect. The wait time is needed so the Pension Office can complete the deduction accounting.
